Welcome to the neem tree!
Thank you for visiting our website and thinking of becoming our patient. Since choosing your dentist is based on trust and confidence, at the neem tree our aim is to build and strengthen a good relationship and provide a high quality service to all of our patients!
First visit
At your first visit to the neem tree you will be greeted by a member of your dental team. You will be asked to fill out a short registration form and a medical history form in our relaxing waiting room which always smells lovely because of the essential oils we have been burning.
The forms can be downloaded by clicking on medical form.
Background music will be playing but if you have any special requests please ask your Patient Care Manager.
For your safety and comfort it is essential to know if you have any medical conditions or disabilities or if you are on any medication. Please fill in the medical registration form as thoroughly as possible.
